Intake Paperwork
If you can, print out your designated form(s) below and fill them out before your visit. This will help the visit go smoother and provide more time for your consultation itself.
Consultation
Next, you will have a consultation with a doctor/massage therapist to discuss your health-related problems, concerns, and potential care options. This initial visit is designed for us to learn more about you, your condition, and expectations to determine how our care can be customized to meet your goals.
Upon entering the lobby, we will request you to fill out our patient forms, if you haven't done so already. This paperwork provides us with your health history, information on your condition, and lets you share your story with us.
Examination
After your consultation, if you have come for a massage, it will begin.
For chiropractic care, you may undergo a thorough examination tailored to your problem. Standard physical, neurological, orthopedic, postural, and nutritional tests may be performed as well. Rest assured nothing will be done in our office without your consent.
Treatment
After your examination you will be moved to a treatment room for your initial treatment if warranted on your first visit. This may include your adjustments, physiotherapy modalities, and/or soft tissue massage.
